Discharge Planning: What It Means When Therapy Ends

One of the most common questions clients and parents ask is: “How do we know when therapy is over?”

What Is Discharge Planning?

Discharge planning is the process of preparing to end regular therapy sessions. It happens when you and your therapist agree that your goals have been met, or that you’re ready to maintain progress on your own.

It’s not an abrupt ending. Instead, it’s a gradual process where we:

  • Review the progress you’ve made.
  • Celebrate the growth and strengths you’ve built.
  • Identify supports and strategies you can continue using.
  • Talk about what follow-up might look like if needed down the road.

In short, discharge planning makes sure you leave therapy with confidence, not questions.

How Do You Know When It’s Time?

Every client’s journey is unique, but here are some common signs you may be ready for discharge:

  • Goals have been met. The challenges that brought you to therapy have been worked through, and you’ve developed coping tools to manage them.
  • Increased confidence. You (or your child) feel more capable of handling emotions, relationships, and stressors.
  • Improved functioning. Symptoms have lessened, and day-to-day life feels more manageable.
  • Consistency over time. Positive changes have lasted, not just for a week or two, but for several sessions in a row.
  • Decreased need. You find yourself relying less on sessions and more on your own strategies.

For children and teens, readiness often shows up as improved behavior at home and school, stronger communication, and fewer emotional outbursts. For adults, it may look like managing stress more effectively, finding joy again, or feeling at peace with challenges that once felt overwhelming.

Why Ending Therapy Is a Positive Milestone

It’s natural to feel nervous about ending something that has been helpful. But discharge isn’t a failure, it’s a sign of success. Therapy has done its job when you no longer need to come every week to feel supported.

Here’s why discharge is something to celebrate:

Life is full of ups and downs, and sometimes challenges resurface after therapy ends. Discharge doesn’t mean you can’t ever return. Many clients check back in with us during a new life transition or stressful season.
